We are looking to recruit a suitably skilled and committed person to support tenants with mental health problems to develop self-management skills, independence and recover meaningful and productive lives in the community.
The post holder will work 3 days per week (Mon-Wed), supporting 9 tenants across 4 supported housing projects. Tenants will be supported around mental health challenges, health and wellbeing, tenancy management and sustainment, managing finances and benefits, daily living skills, plans and aspirations and other dependency issues.
Needs and risks in these areas will be identified through joint working with tenants, involving other professionals and devising a SMART step-by-step, holistic support/action plan to meet goals and targets set. The officer will monitor tenant’s progress through weekly support/key working meetings held in tenant’s accommodation or designated office space. The support officer will ensure all related admin, notes and tenants paperwork are kept up-to-date within standards and in compliance with policies and procedures.
The support officer is expected to attend clinical/professional meetings with tenants, liaising and negotiating on their behalf.
